I have the same form in multiple places on my site. When I use it in a modal I get error messages but when I use it on the “Contact” page it works fine, except that the emails are marked as “read” when I see them in my gmail. I use a google workspace business account and have the app password set up so the form works fine except via the modal. The modal is triggered by the “Schedule Appointment” button in the top bar, footer, or banner.
I noticed that you have the same Gmail App Password between the two usernames. That might be ok, but maybe Google doesn’t like that.
Can you try changing the username on the modals to the same as you have on the Contact page and see if that works?
Also, you’ll probably want to change your Gmail App Password that you are using for the form, as you’ve now publicly posted it on the public forum for 16 hours, so anybody that’s checked your project file has it.
