Informationen zum Kind
- Vorname
- Nachname
- Geburtsdatum
Weitere Informationen
- Name der abgebenden Schule
- Ort der abgebenden Grundschule
Außerdem soll es ein Feld geben, in dem man angeben kann, ob das Kind Geschwister hat. Hier soll man zwischen Ja und Nein wählen können.
Zum Schluss möchte ich zwei mögliche Termine anbieten, von denen nur einer ausgewählt werden kann.
Wenn das Formular abgesendet wird, soll der jeweilige Elternteil anschließend eine entsprechende Bestätigung erhalten.
Bei 1LD gab es dafür früher Super Forms 2. Kann mir jemand erklären, wie ich ein solches Formular mit Elementserstellen und umsetzen kann?
