Post Office Stack - does it allow people to unsubscribe?

I have watched the Post Office stack videos from JoeW and have a couple of questions:

Do I need to place a second stack: one for for people to subscribe and another to unsubscribe?

As I’m using my own database, please explain what the following field means:
