Digitally filling a form

Hello,

I have some questionnaires that my clients are required to fill in prior the consultation; I would like to make their life easier, and I was wondering if there is a way that allows people to fill the questionnaire directly in the file without having to print the file and scanning it.

Any ideas?

Of course. There are plenty of ways to do this but I use FormsPlus from Chillidog.

Hi,
You can do that with Rapidweaver. But I suggest you look as a backend (where data is stored) at something like Google Forms, where a normal spreadsheet is displayed as a “Form”. This form then you can embedd in Rapidweaver or any other webdesign.
Look at Airtable.com too…I suggest GoogleForms however because it is free.

Machform is another option. It also can turn the online form into a PDF automatically

I know you are a Foundation user, the form stacks that come with Foundation should allow you to do this too.

Hi all,

Thank you so much for your help and suggestions.
What I am looking for is NOT something that will be on my site, but it’s like a file/document that I send to people.
I don’t actually need the form to be online. It should a offline file that contain a questionnaire: at the moment I have it in PDF and Word doc.

I am not keen in using the Word document because people can modify stuff and it can be a bit messy; the pdf is cool, but people cannot write on it. And so they need to print the file, fill it up, scan it and send it back to me.

I was wondering it you know of any way I can do this: where I can send a file as a document, but people can fill the document with informations, without touching my writing.

It would awesome if you have something in mind where I can also apply some design to it, such as my colour branding, logo etc. like in the footer, or at the top for example.

Why don’t you create PDF forms with Adobe Acrobat? They can use the Acrobat Reader to fill the forms and e-mail them to you…
