Form stack for a large form

I need a stack for a large form with about 30 fields, which can be designed absolutely flexible. Upload of one or more documents must be possible, possibility of columns, text and other layout objects between the fields and ideally a Re-Captcha or another, suitable human test.

I will use the stack in a responsive RW-Theme or in Foundry. That hasn’t been determined yet.

FormSnap looks like it has all of the features you need. I’ve checked that it also has file upload options. Column layouts can be done inside the theme/Foundry if you go that route.
