Within Total CMS I want a check list

I would like to create within Total CMS a ‘Form’ with a number of options: e.g.

I offer service x ‘Yes or No’
I offer service y ‘Yes or No’

The ‘yes or no’ can be a tick box a drop-down or any form of a specific answer.

When the public-facing page is displayed it shows the results of the checklist.

Has anyone seen a stack that can do this?

You can definitely do this. There are a few different ways to get this done. Toggles can be used to turn on/off data on the content side. You can use the select admin stack to simply set the values of text on the content side as well.
