Foundation form base with attachment is possible?

It totally worked. I don’t understand why a stack for that isn’t in the Foundation form stacks. It doesn’t have to be some elaborate design; just something simple like this.
Just a guess, but the ability for it to work may be highly dependent on server configuration. If so, then it may not be included due to the amount of support request it would create.
