Need a thorough form solution (foundry 2)

I need to build a page similar to this one which has a few questions, a couple dropdown options, etc.
Pretty much a duplicate.
I’m using Foundry 2 for my site.
Tinkering around with ‘Form’ and ‘Form Pro’ but it doesn’t look like either/both of those will fulfill this need.
However, the ‘Form’ that comes with Foundry 3 does but I can’t rebuild site in Foundry 3.

What are my options here?

PS…Any other stacks or plugins suggestions would be appreciated as well.
