How I can create simple survey? is there free plugin or stack?

I use MachForms: it’s very powerful and flexible. But it ain’t free and probably more than you need. There is FormLoom, which also has a stack buddy that comes with it. I’d search for that first. For simple surveys there may be some very good other options that I’m not thinking of.
